How to recruit a patient

 

On recruitment

1. Please check inclusion and exclusion criteria

2. Obtain consent from their next of kin or personal legal representative.

3. Complete screening form SSF1-3 and randomisation sheet (SSF4).

4. Fax SSF1-4 to Carole Turner on +44 (0) 1223414396

At discharge

5. On discharge from the Neurosurgical Unit, or at death, please fax completed data collection sheets

(CRF1-7) to Carole Turner on +44 (0) 1223 414396

At 6 months

At 5 months post ictus, contact the Patient’s GP to confirm that it is appropriate for the patient to complete a follow up questionnaire.

At 6 months approach the patient either at the hospital (if there is a pre-existing clinic appointment) or by post or telephone.  Request completion of  the  modified Rankin Outcome and SF36 questionnaires (questions 1-11) and forward the completed forms by fax to Carole Turner on +44 (0) 1223 414396

If original consent was by a relative or legal representative, request also that the patient completes a patient consent form at this time.
